		<description>Passports have been all the buzz of the travel industry since January 23, 2007.  It is nice to see that others are sharing information on passports.  

I wanted to clarify one point. You said that you must have a notarized consent form.  That is the case only if one of the parents is not able to be present.  Also note that if only one parents is listed on the birth certificate the parent not listed does not need to be present.  

There are thousands of different situations born abroad, divorce, separation, etc.  To outline every situation would be virtually impossible as a comment.
